Becoming a Member
Applicants for membership at Fountain House must meet the following criteria:
1. have a major mental illness (bipolar disorder, schizophrenia or major depression)
2. be at least 16 years of age
3. be clean & sober for at least 90 days
4. must live in the greater New York metropolitan area
5. have a desire to be an active participant in a supportive, vital community
In order to apply for membership, four documents must be completed and sent with an application package to the Membership Office at Fountain House:
1. a completed application, CLICK HERE
2. a current psychiatric evaluation,
3. a detailed, up-to-date psychosocial history
4. a signed release
If you have a specific question about the application materials or process, or would like to have an application package mailed to you, please contact the Fountain House Intake Office at (212) 582-0340 extensions 236 or 240, 9 am to 5 pm EST, and we will be happy to help you.
Please note that we do not accept direct housing referrals. Fountain House housing is available to our members.

Because the number of applications received in recent months has been exceeding our capacity to accommodate new members in the program, we are not accepting applications for new members until further notice.
There are two exceptions to this: a) people aged 16 - 30, who comprise our young adult services program and b) former members of Fountain House who are applying to return.
This situation will be evaluated daily, and we will reopen the intake process as soon as possible. We regret having to take this action, but note that it is done out of fairness to both current and new members. Thank you.
